Be sure to have all paperwork filled out completely online or with you BEFORE you arrive to save time.  If you cannot fill out the forms online and cannot print the forms before you come in just inform the receptionist when you call to make your appointment and they will have them at the office for you to fill out there. (It will take you approximately 20 minutes to fill them out so come in earlier than your exam time).
